New Patient Paperwork
When you schedule your first appointment, we will get information to email and/or text you new patient paperwork to fill out online. If you have trouble getting that done, please print and complete the paperwork below, or use the QR code to fill the paperwork out online. If you would like to fill out the paperwork while at our office, please arrive 20 minutes prior to your scheduled appointment time.
What should I expect at my first appointment?
Your first appointment will include an initial discussion with the Doctor, allowing you to present and discuss your dental needs and concerns. Immediately following your initial discussion, the Doctor will perform a comprehensive oral examination. This examination will include the following:
- Head and neck cancer screening
- TMJ evaluation
- Dental, periodontal, and occlusal evaluation
- Esthetic evaluation
- Photos and any needed x-rays/radiographs
Depending on the extent of the Doctor’s findings, an initial determination will be presented and discussed. Your specific treatment options are customized specifically for your needs and will be presented at a subsequent consultation visit.
Complex dental treatment often requires additional information in order to engineer a treatment plan that is predictable and definitive. The treatment plan may not be presented until all needed information and treatment coordination is completed. Any additional information deemed essential to properly developing a treatment plan will be determined during the treatment presentation.
