Scheduling your first appointment

Call our office at 208-376-0567, where our friendly staff will help you coordinate your initial visit. They will also be glad to answer any questions you may have regarding your first appointment and help ensure you know what to expect.

To help your first visit be as smoothly as possible, please complete and print the HIPAA Agreement, Medical History, New Patient Information, and Financial Agreement Forms prior to your visit. If you desire, you may fax the completed forms to our office prior to your scheduled appointment at 208-376-0661. If you choose to wait or need help filling out the forms at your first visit, please arrive about 25 minutes prior to your scheduled appointment in order to provide adequate time to complete the documents.

What should I expect at my first appointment?

Your first appointment will include an initial discussion with the Doctor, which will provide you with an opportunity to present and discuss your dental needs and concerns. Immediately following the short discussion, the Doctor will perform a comprehensive oral examination. This examination will include the following:

  • A head and neck cancer screening
  • TMJ evaluation
  • A dental, periodontal, and an occlusal evaluation
  • Aesthetic evaluation
  • Photos and any needed x-rays/radio-graphs

Depending on the extent of the findings, an initial determination will be presented and discussed. Your specific treatment options are customized specifically for you and will be presented at a subsequent consultation visit.

Complex dental treatment often requires additional information to engineer a predictable and definitive treatment plan. The definitive treatment plan may not be presented until all needed information and treatment coordination is completed. Any additional information deemed essential to properly developing a treatment plan to satisfy your needs and desires will be determined during the treatment presentation.

Our goal is to engineer a treatment plan that is predictable and successful long term that will ultimately meet or exceed your expectations.


Are there fees associated with the initial examination?

Examination fees will be associated with the initial consultation visit. If you choose not to proceed with treatment in our office, the initial examination fees are non-refundable. If you choose to move forward with your treatment within our office, your initial examination fees will be applied toward your treatment costs.
